Thank you for your post.

To revert back to your maiden name we would require your original document (decree nisi absolute) to be sent into our office.

Also please ensure you write a covering letter confirming that you would like to change your name and sign the letter with both your original signature in the name we have on file and also your new signature if this has changed.

Please also note that we will return the documents in the same form it has been sent in e.g. if you send it in 2nd class we will return it 2nd class, if you send it special/recorded delivery we will return this special/recorded delivery.
