I have just noticed that I was charged a late payment fee for missing a payment (yesterday) even though I had already set up a direct debit using the online app? Why has this happened? Is there something wrong with the system?

I'm really sorry to hear this.
There could be numerous reason why this was charged and why the direct debit didn't collect.
When you set it up, it should come up with a message that will let you know if the direct debit will be active in time for your next payment. If it isn't going to be active, it will suggest making a manual payment for that statement.
