My wife is an additional card holder for our credit card. How can she authorise payments? She has tried to register for the app but it does not recognise her details.

Only primary credit card holders can use Online Banking. Additional cardholders can't access the credit card account online, I'm afraid.
If we have your wife's up to date mobile number, she will be able to approve online transactions via SMS.
